Mentor Agreement

By submitting your Mentor Application, you are consenting to adhere to the policies and procedures described here, and are certifying that all statements below are true and accurate. The “Program” refers to The Mentoring Project.

 

Program Obligations

I agree to serve as a mentor for at least one academic term. I understand that my participation in the Program is voluntary and is administered at the behest of volunteer staff at The Mentoring Project.

I agree to:

  • Communicate with my mentee weekly by electronic methods and to be punctual and responsible about arranging and honoring meeting times.

  • Participate monthly in a one hour-long mentor development training and a one hour-long peer mentor discussion group.

  • Maintain a record of the outcome of my contact(s) with my mentee and to report this data to the Program monthly.

I understand that discussions between me and my mentee are considered confidential and that I will ask my mentee permission to share their information or to inform someone else about a concern/conversation. I understand that serious and life-threatening issues like suicide, substance abuse, mental or physical abuse, domestic violence, and/or criminal activity are best handled by professionals. Even though I might not be considered a mandatory reporter under Washington State Law (RCW 74.34) or any other state law, I will report the suspected abuse, abandonment, neglect, or financial exploitation of a vulnerable adult.

I agree to reserve judgement about my mentee’s personal practices and beliefs, be accepting of differences and not discriminate based on race, color, religion (creed), gender expression, age, national origin (ancestry), disability, marital status, sexual orientation or military status.

I agree to limit contact with my mentee to within the scope of activities conducted by The Mentoring Project and I understand that any relationships or contacts between the mentor and mentee outside of Program activities are neither condoned nor encouraged by The Mentoring Project.

Personal Background

I have not been convicted within the past ten years of any felony or misdemeanor classified as an offense against a person or family, of public indecency, or a violation involving a state or federally controlled substance and that I am not under current indictment.

I understand that my participation as a mentor is contingent on approval by Program coordinators of my application and as part of this application I consent to a background check.

I hereby fully release, discharge and hold harmless The Mentoring Project, participating organizations (including without limitation) and all of its foregoing employees, officers, directors, and coordinators from any and all liability, claims, causes of action, costs and expenses, arising from, relating to or which may be, or may at anytime hereafter become attributable to my participation in the Program.
